自1985年開始，大批科學家湧向偏遠的富產珍珠的卡特哈克島，在此進行各種試驗研究。直到1988年，致命流感在世界範圍內蔓延，為了拯救人類，科學家們召集一群志願者在卡特哈克島進行轉基因疫苗的試驗。他們將一種寄生蟲注入人類體內以增強體質，試驗雖然初見成效，但大多數參與試驗的人員都死於非命。

傑米·阿克曼（Mircea Monroe 飾）是這場災難的少數倖存者之一。20年後，為了處理父輩的遺產，傑米和男友、兄長再次返回這個充滿了恐怖回憶的小島。有的人希望通過她找到殺死寄生蟲的辦法，而與此同時，最新變種寄生蟲正在肆意蔓延，島民的生命受到更大的威脅……
